Health History Update - Used by the Wellness Center for returning athlete updates. If you have an annual checkup with your own physician over the summer and it is within 6 months of the beginning of your sports practices as required by the NCAA, you can have them complete this form and return to us in place of having one performed by the Wellness Center.
