Insurance Procedures and Claims Processing
All student athletes are required to show proof of health insurance coverage. The process of purchasing the College-sponsored student accident and sickness policy, a discussion of the insurance coverage process, and claim forms can be found here.
It is college policy that all full time enrolled undergraduate students have insurance coverage. If you do not have health insurance coverage, St. John Fisher College has established a relationship with The Allen J. Flood Companies to provide an Accident and Sickness Insurance Plan. The plan brochure, explaining eligibility and coverage, is available on the
St. John Fisher College Wellness Center website. If you have questions please contact Angela French at Allen J. Flood at 1-800-734-9326 Ext. 225. You can sign up for coverage on the previously mentioned link to the College’s Wellness Center. Click on the “Accident and Sickness Insurance Plan” link and follow the steps outlined on the website. The policy is effective from August 1
st in the year purchase until July 31
st the following year. When you sign up for coverage your student account will be charged $520.00 in the fall semester for the full year’s premium.
The athletic training staff requires proof of current health insurance for all student athletes. This form is included in the yearly mailing we send to all athletes in the early summer. The form below must be completed and returned to the athletic training department on a yearly basis by August 1st. A front and back copy of the insurance card is required when initially completing the form and if there is a change in insurance coverage. If your coverage is the same from one year to the next a copy of the insurance card is not necessary as the form itself will verify your coverage.
All student-athletes are encouraged to purchase the Student Accident and Sickness Policy
even if they are covered under another policy. In the event of an athletic related injury there will be a coordination of benefits between the two policies and any costs associated with an athletic injury will be covered per the insurance coverage guidelines. The student policy provides coverage up to a $2,000.00 maximum at which point the College’s Sports Injury Policy will provide benefits.
St. John Fisher College’s Sports Injury Policy is for all intercollegiate and club student-athletes. This is an excess policy that has a $2,000.00 deductible and is intended to supplement primary coverage. With the proper completion of a claim form and copies of explanation of benefits (EOB’s) from your primary insurance carrier, you may be reimbursed for any uncovered expenses up to $75,000. If you choose to waive the student coverage and use your own coverage as the primary insurance, the college’s policy will only provide benefits after you have met the $2,000.00 deductible per injury. This policy does not cover intramural sport injuries.
